WAEH supports AAEECE-initiative

The WAEH supports the Worldwide Patient Safety Initiative to address LASA Eye and Ear related Medication Errors of the American Association of Eye and Ear Centers of Excellence (AAEECE).
 
The LASA (Look – alike – Sound – alike) Issue: some examples

**EXAMPLES OF LOOK-ALIKE OPHTHALMIC MEDICATIONS**

  • In an operating room a nurse mistakenly selects a cyclopentolate 1% 2mL ophthalmic solution instead of a tropicamide ophthalmic solution 3mL from a standardized dilation tray.
  • A physician dispenses a 2.5 mL diclofenac 0.1% solution instead of a flurbiprofen 0.03% 2.5mL solution to her patient in her medical office.

These are just some examples of ophthalmic medication related LASA medication errors that occur on a daily basis worldwide in hospitals, pharmacies, physician offices, ambulatory clinics, or in a patient’s home.
 
Worldwide Patient Safety Initiative
 
In an effort to directly address these medication safety issues that affect our hospitals, physicians, nurses, pharmacists, and patients on a daily basis, the AAEECE strongly recommends that pharmaceutical manufacturers address all issues that contribute to existing LASA eye drop related medication errors.

The goal of the AAEECE is not to support the solution that is the most popular but instead to continue to raise public awareness, to share feedback and collect more data from supporters and patients worldwide, and then to systematically evaluate which proposed improvements have the potential for application and benefit.
